Everyday support,given with careand grace.
We help NDIS participants live the life they choose. At home, out in the community, and on their own terms, with the same friendly faces each week.

Supporting NDIS participants across [CITY]

01About
About Hope & Grace
We called ourselves Hope & Grace because that is what good support should feel like. Hope that the week ahead holds something you want to do. Grace in the way it is done: patiently, without fuss, and always on your terms.

A small, local team
We keep our team small on purpose. It means we know every participant by name, and their family too. When you call, you talk to someone who already knows you.

03How we work
Four simple steps,
at your pace.
Starting support should feel easy. You can stop at any step to ask questions, and bring anyone you like along.
Step 01
Get in touch
Call us, send a referral, or ask your support coordinator to reach out. We call back within one business day.
Step 02
Meet & greet
We visit you at home or somewhere you feel comfortable, and listen to what a good week looks like for you.
Step 03
Build your service agreement
Together we write down the supports, times and costs in plain English. Nothing starts until you are happy.
Step 04
Start your support with a matched worker
We introduce a worker who suits your needs and interests. If the match is not right, we change it.

04Why us
What we hold
ourselves to.
- Dignity
- You are the expert in your own life, and we treat you that way at every visit.
- Choice & Control
- You decide who supports you, when and how. We follow your lead.
- Consistency
- The same small team each week, so you are never explaining yourself to a new face.
- Cultural Safety
- We respect your culture, language, faith and identity, and ask how you want them reflected in your support.
05Funding
However your plan is managed, we can help.
Our prices follow the NDIS Pricing Arrangements and Price Limits, so you never pay more than the NDIS allows. Every cost is written into your service agreement before support begins.
Plan-managed
Your plan manager pays our invoices from your NDIS funding. We send invoices straight to them, so there is nothing for you to chase.
Self-managed
You pay us and claim it back from the NDIS. Our invoices are clear and include the right support item numbers.
NDIA-managed
As a registered provider, we claim directly through the NDIS portal. You do not handle any payments.
Ask us for our current price list. We are happy to go through it with you.
06Stories
What people say about us
Before Hope & Grace we had a different worker almost every week. Now Liam has the same two people, and they really know him. He looks forward to Tuesdays again.
