Privacy Policy
Last updated [DATE]
Your privacy matters to us. This policy explains what personal information Hope & Grace Care Services collects, why we collect it, and how we keep it safe. We follow the Australian Privacy Principles in the Privacy Act 1988 and the NDIS Code of Conduct.
What we collect
- Contact details, such as your name, phone number, email and address.
- NDIS details, such as your NDIS number, plan dates and funded supports, when you choose to share them.
- Information about your health, disability and support needs, so we can provide safe and suitable support.
- Details of family members, carers or support coordinators you ask us to stay in touch with.
How we use it
We use your information to respond to referrals, plan and deliver your supports, write your service agreement, send invoices and meet our obligations as an NDIS provider. We do not sell your information, and we do not use it for marketing without your permission.
Who we share it with
We only share information with your consent, or where the law requires it. This may include your plan manager, support coordinator, the NDIA or the NDIS Quality and Safeguards Commission.
Keeping it safe
Records are stored securely, and only staff who need them can see them. We keep records for as long as the law requires, then destroy them securely.
Seeing or correcting your information
You can ask to see the information we hold about you, or ask us to correct it, at any time. Call us on 0400 000 000 or email hello@hopeandgracecare.com.au.
Concerns
If you are worried about how we have handled your information, please tell us first so we can put it right. You can also contact the Office of the Australian Information Commissioner at oaic.gov.au.
